Apply to become a member of the Kentucky Dental Association today by downloading a membership application form. 

Complete the membership application and mail it to the KDA offices at 1920 Nelson Miller Parkway, Louisville, KY 40223.. Once the application is recieved, a KDA representative will contact you. If you have any questions, please call us at 502-489-9121.
